NEW PATIENTS PRIOR TO YOUR VISIT

1.  Please check to see if I am in your particular insurance network. 
Sorry I am not taking new Medicare or OHP/Medicaid patients at this time.

2.  To make an appointment click on the green tab  "PATIENT PORTAL" above.

3.  In the Portal click on New Patient Registration 
   
4. After you register, go to questionnaires to complete the following:
A.    HIPPA Privacy Authorization Form 
B.    General Consent to Treatment 
C.    Medical History Questionnaire IFM for adults  or  Pediatric Intake Form
D.    Social History Questionnaire
4. Then you may request an appointment.

FOR ESTABLISHED PATIENTS

1.  To make an appointment click on the green tab  "PATIENT PORTAL" above.

3.  In the Portal click on New Patient Registration
     (this applies to existing patients that are new to the portal, however this will not work if earlier you received an email inviting you to the portal.  If you got that email & didn't respond then, now  choose "Create Patient Portal Account" (for already registered patients)-as I partially started the registration for you.
